    DTI-ALPS联合gBOLD-CSF技术评估偏头痛患者类淋巴系统功能

    Assessment of Glymphatic System Function in Migraine Patients Using DTI-ALPS Combined with gBOLD-CSF Technique

    • 摘要: 目的:探讨无先兆偏头痛(MwoA)患者大脑类淋巴系统功能的变化及其与临床指标的相关性。方法: 招募50名MwoA患者及49例匹配的健康对照组。采用3.0T MRI获取影像数据,计算两组沿血管周围间隙的扩散张量成像分析(DTI-ALPS)指数及全局血氧水平依赖性信号与脑脊液流入动力学之间的耦合强度(gBOLD-CSF)的差异,进而评估MwoA患者类淋巴系统的功能变化,并分析其与临床量表评分的相关性。结果:相比健康对照组,MwoA患者左侧DTI-ALPS指数、gBOLD-CSF及-d(gBOLD)/dt-CSF耦合强度明显减低,且患者左侧ALPS指数与贝克焦虑量表评分呈弱负相关。结论:MwoA患者存在大脑类淋巴系统功能障碍,其异常可能参与偏头痛的病理生理过程并与焦虑症状相关,为深入理解偏头痛的发病机制提供新视角。

       

      Abstract: Objective: To investigate alterations in glymphatic system function and correlate them with clinical indicators in patients with migraine without aura (MwoA). Methods: A total of 50 MwoA patients and 49 matched healthy controls (HC) were enrolled. Imaging data were acquired using 3.0T MRI. The differences in diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index and the coupling strength of global blood-oxygen-level-dependent signal and cerebrospinal fluid (CSF) inflow dynamics (gBOLD-CSF) were calculated between the two groups to evaluate the glymphatic system function in MwoA patients; their correlations with clinical scale scores were further analyzed. Results: Compared with healthy controls, MwoA patients exhibited significantly lower left-sided DTI-ALPS index, reduced gBOLD-CSF coupling strength, and diminished -d (gBOLD)/dt-CSF coupling strength. Additionally, the left-sided ALPS index was negatively correlated with the Beck Anxiety Inventory score. Conclusion: MwoA patients demonstrate impaired glymphatic system function, which may contribute to the pathophysiology of migraine and be associated with anxiety symptoms. These findings provide new insights into the mechanisms underlying migraine.
