体型因素及组织成分对成人胸部CT水当量直径的体型特异性剂量估算的影响

Impact of Individual Body Shape Factors and Tissue Contents on the Water Equivalent Diameter of Size-Specific Dose Estimations in Adult Chest CT

  • 摘要: 目的:探讨个体的体型因素及组织成分对成人胸部水当量直径的体型特异性剂量估算(SSDEWED)的影响。方法:收集在本院行胸部CT平扫的成人受检者205例,采集其个体信息(身高、体重、身体质量指数);在中间层面的横断位图像上测量胸廓的平均CTROI值、横断面积AROI、横径、前后径;测量整个扫描范围的脂肪含量、脂肪体积和肌肉含量、肌肉体积;计算前后径/横径、扫描长度、扫描长度/横径;获取受检者的容积CT剂量指数(CTDIvol),计算出所有受检者中间层面的SSDEWED的数值。分析脂肪含量、肌肉含量、脂肪体积、肌肉体积、BMI、横断面AROI、横径、前后径、扫描长度、扫描长度/横径比、前后径/横径比与SSDEWED的相关性。结果:脂肪含量、脂肪体积、肌肉体积、BMI、横断面AROI、横径、前后径、扫描长度/横径比与SSDEWED有显著正相关性;肌肉含量、前后径/横径比值、扫描长度与SSDEWED无相关性。结论:多种个体体型因素及组织成分与SSDEWED有显著相关性,其中脂肪组织是影响SSDEWED的关键的因素。

     

    Abstract: Objective: To investigate the impact of individual body size factors and tissue composition on the size-specific dose estimate(SSDEWED) based on water equivalent diameter for adult chest. Methods: A total of 205 adult subjects who underwent chest CT scans at our hospital were enrolled, and their individual information (height, weight, body mass index) was collected. The average CTROI value, cross-sectional area AROI, transverse diameter, and anteroposterior diameter of the thorax on the axial images at the intermediate level were collected. The fat content, fat volume, muscle content, and muscle volume across the entire scanning range were recorded. The anteroposterior diameter/transverse diameter, scan length, and scan length/transverse diameter were calculated. The volume CT dose index (CTDIvol) of the subjects was obtained, and the SSDEWED values at the mid-level of all subjects were calculated. The correlations between fat content, muscle content, fat volume, muscle volume, BMI, cross-sectional AROI, transverse diameter, anteroposterior diameter, scan length, scan length/transverse diameter ratio, anteroposterior diameter/transverse diameter ratio, and SSDEWED were analyzed. Results: Fat content, fat volume, muscle volume, BMI, cross-sectional AROI, transverse diameter, anteroposterior diameter, scan length/transverse diameter ratio are significantly positively correlated with SSDEWED. Muscle content, anteroposterior/transverse diameter ratio, and scan length showed no correlation with SSDEWED. Conclusion: Multiple individual body size factors and tissue compositions are significantly correlated with SSDEWED, among which adipose tissue is the key factor influencing SSDEWED.
