对比增强磁敏感加权成像对脑海绵状血管瘤的成像价值

Imaging Value of Contrast-enhanced Susceptibility-weighted Imaging for Cerebral Cavernous Malformations

  • 摘要: 目的:联合应用对比增强磁敏感加权成像(CE-SWAN)和磁敏感加权血管成像(SWAN),评价CE-SWAN对脑海绵状血管瘤(CCM)基本特征的显示价值。方法:顺序收集CCM患者27例,男性15例,女性12例,平均年龄39.7岁(15~64岁)。行常规磁共振平扫、SWAN和CE-SWAN。SWAN中幅度图和相位图独立生成,对CCM从数量、结构及分型、周围引流血管3个方面进行评价。将图像对CCM特征的显示效果从低到高分为4个等级,分别由低至高赋予1至4的分值,由两名经验丰富的中枢神经系统影像诊断医师逐个量化评价各个图像对CCM特征的显示效果。所有数据采用SPSS 17.0统计分析软件处理,编秩后采用单因素方差分析进行评价,P<0.05认为差异具有统计学意义。结果:CE-SWAN对CCM的检出率及其结构的显示与SWAN无明显差异;对周围引流血管的显示具有显著统计学差异。结论:CE-SWAN对CCM周围引流静脉的显示优于SWAN,是传统磁共振成像序列的有效补充,具有评价脑出血转归过程的潜在价值。

     

    Abstract: Objective: To compare the contrast-enhanced susceptibility-weighted angiography(CE-SWAN) and susceptibility-weighted angiography(SWAN), and to evaluate the value of CE-SWAN imaging for the basic features of cerebral cavernous malformations(CCM). Methods: Twenty-seven patients with CCM were collected, including 15 males and 12 females while an average age of 39.7 years(15 ~ 64 years). They underwent conventional MR, SWAN with/without contrast enhancement MR scan. In the magnetic SWAN, magnitude map and phase map were generated independently, and the CCM was evaluated from three aspects: number, structure and typing, surrounding blood vessels. The displaying effect of the image on the characteristics of CCM is divided into four grades from low to high, and the scores of 1 to 4 are assigned from low to high, respectively. Two experienced central nervous system radiologists quantitatively evaluate each image one by one. The displaying effect on the characteristics of CCM. All data were processed by SPSS 17.0 statistical analysis software, and the rank was evaluated by McNemer-Bowker Test. P<0.05 was considered to be statistically significant. Results: The CE-SWAN sequence showed no significant difference in the detection rate and structure of the CCM with SWAN. There was significant statistical difference in the peripheral drainage vessels. Conclusion: CE-SWAN is superior to SWAN in the detection of drainage veins around the CCM. Which is an effective complement to traditional magnetic resonance imaging sequences and has potential value in evaluating the outcome of cerebral hemorrhage.
